NEW ELECTRONIC REGISTRATION PROCEDURE OPTION
ELECTRONIC REGISTRATION PROCEDURES
All documents MUST be scanned and sent as an Adobe PDF file (NO EXCEPTIONS). Go to your local copy store and
have them save it as a PDF Field. When scanning, make it sure it is in the lowest resolution (DPI).
1. Send official stamped roster.
2. Send file with player passes including guest players and guest player forms in alphabetical order and only
the side with their name and DOB (only one side of pass required). 2011/12 US Youth Passes are valid
through August 31, 2012.
3. Send file of your medical release forms including guest players in alphabetical order.
4. Send file with Permission to Travel. *Only required from teams attending as US Youth Soccer and coming
from Connecticut, New Jersey or outside of Region I.
The file should be saved as your Current Age, Gender, Club and Team Name. (i.e. U14FWCUSCPredators)
6. One will receive email confirmation once registered.

GIRLS U12-U14 PLAYING AT POLK VALLEY MUST STILL PICK UP THEIR PARKING PASSES AT COUNTY FIELDS. SEE BELOW FOR TIMES AND LOCATIONS.
The OTHERWISE MANDATORY tournament registration will be held on Friday, September 2 at Lehigh County Sports Fields Pavilion from 5:00-6:00pm, 6:30-9:00pm or Saturday, September 3 at the Lehigh County Sports Fields LVU Spirit Wear tent at least 1 hour before your first game.

The Lou Ramos Classic is a US Youth Soccer sanctioned tournament that accepts US Club Soccer teams. You will need to bring the following items to Registration:
US Youth Soccer Teams
1. Original State Approved Roster and 1 copy for the tournament to keep
2. Validated US Youth Soccer Player Passes (2009/10 passes are valid through August 31, 2010)
3. Current, signed Medical Release for every player – Forms DO NOT have to be notarized.
4. Permission To Travel – Region I teams, with the exception of Connecticut and New Jersey, do not require Permission to Travel. Connecticut and New Jersey teams must provide Permission to Travel. All other teams that are not from Region I must provide Permission to Travel.
5. Guest Players – Player Pass, Medical Release and Guest Player forms as required by your state association.
The Guest Player Form can be found on most State organizations’ websites. Several have it set up to do
electronically and it only takes 5 minutes. If your state has it, it will be listed as E-Travel. Just select “Guest Player Form”. For your reference, I have listed the states that offer E-Travel below.

US Club Teams
1. US Club Roster and 1 Copy for the tournament to keep
2. Valid US Club Player Passes (You will need to use the NEW 2009/10 passes)
3. Current US Club signed Medical Release forms for every player.
Teams may have no more than 18 players (U09, U10 and U11 (8v8) – no more than 14). Three Guest Players per team will be permitted for all age groups.
US Club Soccer teams may only take guest players registered under US Club Soccer, and US Youth Soccer teams may only take guest players registered with properly stamped US Youth Soccer player pass cards.
